The patient has an illness or injury that makes leaving home difficult, requires assistance from others or supportive devices, or is medically contraindicated.
The patient resides in our active service area. (Contact our office with your ZIP code to verify travel eligibility).
We accept Original Medicare and select Medicare Advantage plans. Coverage and eligibility are verified before your first visit. Patients with Medicare Supplement coverage should provide their secondary insurance information during intake.
