Your plan fee covers only the services specifically listed as included. Emergency care, hospitalization, specialist care, medications, laboratory and imaging services, and other services outside the plan are not included and may result in additional costs.
The Individual Health & Wellness Plan is a self-pay program, with select insurance plans accepted for eligible healthcare services provided separately from the plan.

Additional scheduled follow-up visits beyond the four included visits are $75 per visit.
