  • Annual Physical Exam: One complete annual check-up evaluating history, vitals, and system reviews.
  • Electrocardiogram (EKG): Basic heart rhythm screening performed in-office.
  • Point-of-Care Testing: Instant checks for Glycated Hemoglobin (A1C), Blood Glucose, Urinalysis, and Lipid/Cholesterol profiles.
  • Up to 4 Scheduled Follow-Ups: Convenient consultation visits per year, available in person or via telehealth.
  • Acupuncture Wellness Session: One complimentary session with our affiliated licensed acupuncturist.
  • Behavioral Health Consultation: One introductory virtual session with our affiliated mental health partner.
  • Discounted Diagnostics Network: Special pre-negotiated pricing for advanced laboratory work and imaging with third-party providers.

⚠️ What is NOT Included

To maintain safety and regulatory compliance under New York State guidelines, the following acute, emergency, and complex clinical services are excluded:

  • ✕ Emergency Care: Evaluation of medical emergencies, including severe chest pain, respiratory distress, acute trauma, or suspected fractures.
  • ✕ Urgent Care: Suturing lacerations, casting, or advanced IV fluid therapies.
  • ✕ Specialist Visits: Consultations with cardiologists, oncologists, or surgeons.
  • ✕ Inpatient/Hospital Stays: Room, board, and operations within hospital walls.
  • ✕ Advanced Laboratory Panels: Specialized genetic, hormonal, or pathology panels.

Workforce Subscription

$135 / employee / month
Introductory Rate for the first 3 months

Regular rate: $150 per employee per month

  • $600 minimum monthly commitment
  • Annual physical & EKG
  • Available point-of-care testing
  • Up to four follow-up visits annually
  • Complimentary acupuncture and behavioral health sessions
  • No copays or deductibles for included program services
  • No emergency, inpatient, or specialist care

Initial term: 3 months, continuing month-to-month thereafter. Cancellation requires 30 days written notice. Additional follow-up visits beyond the included 4 are billed at $65–$75 per visit.
